Who should never start this use

Other medicines that change this use
Potent CYP3A4 inhibitors - ketoconazole, itraconazole, ritonavir, clarithromycin - raise tadalafil AUC. Daily maximum becomes 2.5 mg. PRN maximum becomes 10 mg once every 72 hours. That is a labelled cap, not a suggestion.
CYP3A4 inducers such as rifampin shave exposure. A man who 'stopped responding' after starting tuberculosis therapy may have a pharmacokinetic problem, not a new vascular one.
